    Real World IOP Monitoring

    Real World IOP Monitoring

    IOP is the only modifiable risk factors for development or progression of glaucoma. It has been shown to be affected by various factors and to fluctuate within a day or over several days or months. In our real-world practice, IOP readings are typically obtained with the patients seated during the office hours every 3 to 6 months.     The Value of 24-hr Home Self-tonometry in Diagnosis and Management of Glaucoma

    Modern home tonometry can provide intraocular pressure measurement results with accuracy, similar to that of Goldmann applanation. Over multiple measurements a patients diurnal pattern or pressure variation can be derived. Multiple measurements also allow the clinician to see true effects of treatment without confounding problems of measurement inaccuracy and regression towards the mean.     IOP is a dynamic parameter showing evidence of glaucoma progression due to IOP peaks and fluctuations (unmasked with 24-hour IOP monitoring). As clinicians, we must define an acceptable range for IOP fluctuations along with the target IOP for an individual. During the prescription of antiglaucoma medications, the aim should be to provide a 24-hour cover with reduced IOP fluctuations.
